In people who are genetically susceptible, DHT gradually causes certain follicles to miniaturize.

Why the Pattern Often Tells More Than the Lab Panel

Many patients arrive with bloodwork that is technically “normal.”


That doesn’t mean the thinning isn’t real.


Hormonal hair loss is often diagnosed by looking at the pattern of follicle change over time, not by finding one abnormal lab value.


Under magnification, a trichologist may see:

  • Empty follicles

  • Miniaturized hairs

  • Variation in hair diameter

  • Short upright regrowing hairs

These clues can help distinguish gradual androgen-related thinning from other types of hair loss.

A familiar scenario:

“I swear my hair fell out overnight.”

We compare today’s scalp with photos from 12–18 months earlier and discover that the center part has been slowly widening the entire time.


The change was gradual enough to go unnoticed until it crossed a visual threshold.


Nothing was truly sudden.


The progression was simply slow enough to hide in plain sight.


Hormonal Thinning vs. Telogen Effluvium

This is one of the most important distinctions.

  • Evenly all over?


How quickly did it happen?

  • Gradually over many months?

  • Noticeably over a few weeks?

The speed of change often provides a major clue about the underlying cause.
